HOW WE WORK
How we work with NDIS participants
STEP 1

Referral
You or your support coordinator can contact us directly. We accept self-referrals and referrals from support coordinators, LACs, case managers and GPs. Complete our online enquiry form or call 07 3666 8700.
STEP 2

Initial assessment
One of our therapists will visit you at home to understand your goals, daily routines and the challenges you're experiencing. We'll review your NDIS plan and work out how we can best support you.
STEP 3

Recommendations & reporting
We prepare detailed reports and recommendations aligned with your NDIS goals. For assistive technology and home modifications, we liaise directly with suppliers and registered tradespeople to make the process as smooth as possible.
STEP 4

Ongoing support
We work collaboratively with you, your carers, and your support network to review progress and adjust our approach as your needs change.
