Department of Veteran's Affairs
DVA Gold and White Card holders are eligible for fully funded services. A D904 referral from your GP is required.
Private Health Insurance and Chronic Disease Program Referrals
GP Management Plans Visits are charged a Gap fee of $40 per visit.
Our Private fees range from $100 - $130 per hour depending on complexity. Extras Private Health Insurance policies will often rebate these costs.

Bayside OT is a registered NDIS provider. Our provider number is 4050037466. We are registered to provide services for clients needing Assistive Technology and recommendations for Minor Home Modifications.
Nursing Home Referrals
We charge $100 - $130 per hour depending on complexity.
