Bayside OT is an established provider of Occupational Therapy services though the Department of Veteran's Affairs Rehabilitation Appliances Program.
All of our therapists are highly experienced and able to provide equipment and arrange services to enable retired servicemen and women and war widows/widowers to live safely and independently.
We require a medical referral called a D904 from a GP, specialist or hospital discharge planner in order to commence our intervention. Please contact us if any information is required or to discuss a DVA referral.
